- Print, Complete and Sign Our Intake Form

Bring your signed Intake Form along with:

Photo ID such as a driver’s license

Proof of current residence such as a utility bill (We serve Wakefield, Brookfield, Effingham, Milton, Middleton, and Newfields, ME)

Written documentation of current services that you are receiving such as food stamps, fuel assistance, Medicaid. We will require the letter that you receive these services.
- Come to the Food Pantry with your paperwork during our scheduled hours.
